Discussed the center policy on outdated medication, medication authorization forms, allergy action plans, allergy list in each classroom, staff awarness of medication, staff belongings, cleaning supplies storage areas and procedures in place to keep staff cleaning cart attended.

An unannounced monitoring inspection was conducted on May 12, 2022. At the time of entrance there were 73 children and 14 staff members present with childre. Additional staff werle availabe The sample size consisted of 4 children's records and 4 staff records. Children were observed eating snack, center activities. The inperson inspection began at approximately 2:44 pm and ended at approximately 5:00 pm. Records were reviewed virtually. Areas of non-compliance are identified on the violation notice.

Standard #: 8VAC20-780-40-M Description: Based on record review and staff interview, the licensee did not ensure that a current written list of all children allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.
Evidence:
1. The center did not have updated written list of children allergies, dietary restriction and sensitivities in each room where children are present.
2. Infant 1, Infant 2, and 2 year old classroom the allergy list was not dated.

Standard #: 8VAC20-780-280-B Description: Based on observation and staff interview, the licensee did not ensure that all hazardous materials were kept in a locked location.
Evidence:
In the 2 year old classroom , the cabinet over the changing table was unlocked there was bleach solution, sanitizers and soap and water.
In the 2 year old bathroom, there was lotion and hand sanitizer which had two labels keep out of reach of children, caution and warning.Plan of Correction: Not available online. Contact Inspector for more information.

Evidence:
1. In infant 1 classroom, there was one crib which was not labeled with the name of the child using it.

Standard #: 8VAC20-780-500-B Description: Based on observation and staff interview, the licensee did not ensure there was a leakproof covered receptacle for soiled linens.
Evidence: In the 2 year old classroom bathroom, next to both toilets, there were two receptacle that were over flowing of soiled linens. The soiled linen receptacles were not covered. Standard #: 8VAC20-780-560-F Description: Based on observation, the center did not ensure that when centers choose to provide snacks the following shall apply: Centers shall follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).

1. Children were served cinnamon crackers with water for afternoon snack.
2. A second food component was not served to children. USDA requires snack to consist of two separate food components.
