Comments:
An unannounced monitoring inspection was conducted on site on 05/23/2022. The inspector arrived at 8:45 AM and departed at 11:20 A.M. There were 25 children present with 5 staff. The inspector reviewed five children?s records and six staff. The inspector observed the children outside. Discussion with director about the menu.

Standard #: 8VAC20-780-260-A
Description: Based on review of the most recent fire inspection the, center failed to ensure that an annual fire inspection was conducted.

Evidence:
1. The most recent fire inspection was 1/28/2021

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that all areas and equipment was maintained in a clean, safe and operable condition.

Evidence:
1. Observed in the playground area a thorny bush that was approximately 8 inches in diameter. Several children were outside with no shoes on. One child pointed the thorny bush out to the inspector and told her ?that it pokes them and they are not allowed to touch it.?
2. Observed children playing on a large makeshift play structure. This play structure was made of tree logs. The logs were different sizes, some were approximately 6 ft. long with approximately 2 ft. diameter. The logs were placed haphazardly with no ground support to keep them from moving. The entire play structure stood approximately 5 f tall. The staff # 4 stated that the logs are supported by the weight and pressures of the logs. There were several areas that had openings that could allow for children to fall down into the middle of the structure and cause an entrapment hazard or allow for a pinch point for children?s limbs.
The play structure had several protruding limbs and had splintering pieces that posed a hazard.
3. Observed around the middle of the outside play area were small logs used as stepping stones. Some of the logs had limbs protruding out that posed a hazard to the children in care.

Plan of Correction: Protruding branch was cut from log. The children are no longer
allowed to play on log structure. Thorny weed was
removed. Per the director.

Standard #: 8VAC20-780-330-B
Description: Based on observation, the center failed to ensure that all play structures that required resilient surfacing had the required amount.

Evidence:
1. The tire swing had resilient surfacing directly under the swing. The requirement is for the resilient surfacing to extend all directions that the tire swing goes and double the area.
2. The makeshift log play structure did not have the required resilient surfacing around the perimeter. There was not enough mulch around it to measure.

Plan of Correction: We have added the required amount of mats under the swings. Children are no longer
allowed on the log structure 5/31/22

Standard #: 8VAC20-780-340-A
Description: Based on observation and interviews, the center failed to ensure that when staff are supervising children they always ensure their care, protection and guidance.

Evidence:
1. There were several child sized picnic tables in the outside play area. Approximately six chickens were observed roaming around the play area with the children and were observed walking on the picnic tables. The children were asked to remove the chickens off the picnic tables. Also while the chickens were roaming around there were children approximately ages 14 months ? 5 years were playing in the same area. The 14 month old child was observed crawling on the ground and two children were walking around the play area barefoot.
2. Observed three goats in a corral inside the play area. The goats were able to place their heads through the gate and have access to the children. Inspectors observed children feeding the goat weeds from the play area. One child stated to the inspector that ?the goats bite our fingers?. Staff # 4 was asked about the goats and she stated that ?the goats nibble on the children?s? clothes.
3. When Licensing staff entered the infant room, a small dog started barking and growling and ran across the infant floor beds towards the inspectors. Staff # 3 stated "you startled the dog". The infant beds were not occupied

Standard #: 8VAC20-780-350-C
Description: Based on observation of the outside play area, the center failed to ensure that when children are in ongoing mixed age groups, the staff-to-children ratio is applicable to the youngest child in the group.

Evidence:
1. Upon arrival at 8:45 A.M. there were 16 children outside ages 14 months to 5 years old with two staff. The required ratio is one staff to four children when the group is combined. The required number of staff is four staff.
At 9:08 A.M. there were 22 children with four staff. The required number of staff would be seven for this grouping of children.

Plan of Correction: We are installing additional fencing for the Nido play area

Standard #: 8VAC20-780-440-A
Description: Based on observation and discussion with the director, the center failed to ensure that all children 12 months and younger were using a crib.

Evidence:
1. On the day of the inspection there were three children 12 months and younger that were sleeping on an infant bed and not the required crib per the standard.

Plan of Correction: This is still an on going issue with the state. We will once again submit
a variance request, as the use of floor beds is strongly rooted in Montessori
philosophy. Per the director.

Standard #: 8VAC20-780-500-B
Description: Based on observation of the infant room, the center failed to ensure that when cloth diapers are used a separate leak proof storage system that is either foot-operated or used in such a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface of the storage system.

Evidence:
1. Observed two zipped cloth bags hanging from the diaper changing table. Each bag had soiled cloth diapers in them. The method of disposal did not ensure that a staff?s member?s hands nor the soiled diaper touched the exterior surface of the storage system.

Plan of Correction: Disputed - Under formal review.

Standard #: 8VAC20-780-530-C
Description: Based on review of six staff?s records, the center failed to ensure that at least two staff members who meet the requirement for First Aid and CPR present on the premises during the center?s hours of operation.

Evidence:
Staff # 6 was the only staff present during the inspection that had First Aid and CPR.

Standard #: 8VAC20-780-600-F
Description: Based on interviews and observation, the center failed to ensure that all manure was removed from barns, stables, and corrals at least once a day and stored and disposed of in a manner to prevent the breeding of flies.

Evidence
1. Observed chickens roaming around freely in the outside play area. Verified through interviews the chicken coop is cleaned weekly.
2. There were three goats in a corral that has a fence around it. Verified through interviews the corral is cleaned weekly.
