22.1 BACKGROUND CHECKS CODE; CARBON MONOXIDE

Technical Assistance:
Discussed reviewing the new CDC standards that became effective 10-13-2021 and the new website (Child Care VA) https://www.doe.virginia.gov/cc/.
Clarified requirements for CPR and First Aide training / orientation and certification.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in person tour of the program.
A renewal inspection was initiated on June 1, 2022 and concluded on June 1, 2022. There were 40 children present, ranging in ages from 3 to 5, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Violations:
Standard #: 22.1-289.058
Description: Based on observation and staff interviews, the licensee did not ensure that each building that was built before 2015 and that house a child day program that is licensed and serves preschool-age children shall be equipped with at least one carbon monoxide detector.

Evidence: The Education Manger confirmed they do not have a carbon monoxide detector.

Plan of Correction: The carbon monoxide detector was purchased and installed at the Head Start Poplar Creek location on June 1st. The carbon monoxide detector is plugged into the wall and has battery backup.

Standard #: 8VAC20-780-40-M
Description: Based on record review and staff interviews, the licensee did not ensure the center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions.

Evidence: The Education Manager confirmed there was not a current written list of all children's allergies, sensitivities, and dietary restrictions. The record for child #2 included a diagnosed food allergy (red dye) from a physician. Child #2 was in care during the inspection

Plan of Correction: A child that had a known allergy was not listed on the class allergy list, however, the Education Manager updated the class allergy list, and it is currently posted in the classroom.

Standard #: 8VAC20-780-60-A-8
Description: Based on record review and staff interviews, the licensee did not ensure there was a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The record for child # 2 has a diagnosed food allergy (red dye) and the record did not include a written care plan, to include instructions from a physician.

Plan of Correction: The Wellness Practitioner contacted the parent as well as the student?s physician office to notify them that an Allergy Action Plan was needed. The paperwork was faxed to the physician office on June 1st and is expected to be returned to the Head Start office by June 8th.

Standard #: 8VAC20-780-240-B
Description: Based on record review and staff interviews, the licensee did not ensure staff had complete orientation training that was appropriate to the age of the children in care and include all of the facility specific topics.

Evidence: The Human Resource staff confirmed there was not information available for review confirming staff #4 (date of hire 4-16-2022) had obtained facility specific orientation.

Plan of Correction: The staff member that need additional site-specific orientation training will be completed by June 7th by the Head Start Director. This will be documented in their file. Also, moving forward, the orientation documents will be updated to include all licensing standards and center specific site orientation.

Standard #: 8VAC20-780-240-E
Description: Based on record review and staff interviews, the licensee did not ensure that within 30 days of the first day of employment, staff must complete orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.
Evidence: The record for staff #4 (date of hire 4-16-2022) did not include information regarding CPR and First Aid orientation training.

Plan of Correction: Staff #4 will complete the online training by June 17, 2022 and will be included in on the in person training in August 2022.

Standard #: 8VAC20-780-530-A-1
Description: Based on record review and staff interviews, the licensee did not ensure there was at least one staff in each classroom or area where children are present had obtained a current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care. The training shall include an in-person competency demonstration.

Evidence: There was not a staff member with CPR certification in 3 out of 3 of the classrooms observed during the inspection. Classroom #1 included 14 children, with staff #5 and staff #2. Classroom #2 included 15 children with staff #6 and staff #7. Classroom #3 included 11 children with staff #8 and staff #3. All children were ages 3-5.

Plan of Correction: School will end on June 9th. All staff that only had online certification will be required to complete in-person Infant / Child / Adult CPR, First Aid & AED training before students return on August 29th. Any current new hires will complete an online CPR Training by June 17th and will be included for the in-person training in August 2022.
HR will verify CPR and first aid AED certification during onboarding or have them scheduled for class by their start date.
