Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department. regarding allegations in areas of Staffing and supervision , Programs and Staff Qualifications and training . The inspector was on site May 4, 2022, May 17, 2022 and May 20, 2022. Interviews with staff were conducted, documents were reviewed and observations were made in the center. The preponderance of the evidence gathered during the investigation supports the allegation ; therefore, the complaint is determined to be valid.

Description: Based on record review and staff interview, the licensee did not ensure there was always at least one staff member on duty who has obtained, within the last three years, instruction in performing the daily health observation of children.

Evidence:
Staff #1 stated that Staff #6 is the employee that opens the facility in the morning with Staff #2. After a review of records, it was determined that neither Staff #6, nor Staff #2 have obtained daily health observation training.

Plan of Correction: All staff including openers and closers will obtain the health screening

Description: Based on record review and staff interview the licensee did not ensure during the stated hours of operation, there always shall be on the premises and on field trips when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them. The volunteer or other employee shall have received instruction in how to contact appropriate authorities if there is an emergency.

Evidence:
On May 4, 2022 Staff #1 stated Staff #6 opens the facility. Record reviewed staff #6 does not meet qualifications of a program lead as of May 4. Confirmed with Staff #1.

Complaint related: No
Description: Based on observation and staff interview, the licensee did not ensure that ratio requirements were maintained whenever children are in care.
Evidence:
1. The center received a ratio waiver for one staff to five infants. It was observed on May 17, 2022 between 8:15am- 8:32am, that there were eight children with one staff as confirmed by Staff #1 (person in charge). The youngest child in the group was nine months old.

Description: Based on observation and staff interview, the licensee did not ensure that ratio requirements were maintained whenever children are in care.

1. The center received a ratio waiver for one staff to 11 children three years old up to school age. At
8:47am on May 17, 2022, it was observed that there were 12 children with one staff in the Pre-K
classroom as confirmed by Staff #1.

Plan of Correction: There are two teachers assigned to each classroom and on was off.
Remove kid to balance ratio.

Description: Based on record review, the licensee did not ensure there was a current certification in cardiopulmonary resuscitation (CPR) as appropriate that included an in-person competency demonstration.

Evidence: Staff #1 was working alone with a group of children in the infant classroom. Staff #1 verified, and the record review confirmed, that the CPR training for Staff #1 did not include an in-person competency demonstration.

Staff #6 is the employee that opens the facility in the morning with Staff #2. After a review of records, it was determined that neither Staff #6, nor Staff #2 have obtained current CPR training with an inperson demostration.

All staff will obtain in person Cpr and First Aid
