An unannounced monitoring visit and facility tour was conducted on 5/17/2022. At the time of inspectors' arrival there were 12 infant to preschool aged children in care with 3 staff members. Children were observed interacting with staff, nap, lunch, diapering and hand washing. A sample of 4 children's records and 4 staff records were reviewed. The director reported no medications are being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Standard #: 22VAC40-185-270-A Description: Based on observation it was determined the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. There was peeling paint and exposed drywall in the Infant, Toddler, and Pre-K Classrooms.
2. There was exposed weed barrier cloth on the playground outside.
3. There was a karaoke machine stacked on top of plastic storage drawers that wobbled with the cord dangling down in the Pre-K classroom.
4. There was a broken plastic drawer in the bathroom used by the children in care.
5. Children were observed in the areas noted above with the potential hazards during this inspection.Plan of Correction: We will have maintenance fix all noted issues. We will have staff remove machine and place it on the floor. We will have staff remove broken drawer.
Standard #: 22VAC40-185-290-3 Description: Based on observation it was determined the center did not ensure all electrical outlets in areas where preschool aged children are in care have protective covers.
Evidence:
1. There was an uncapped electrical outlet in the toddler room.
2. Staff confirmed that the outlet was uncapped.Plan of Correction: We will have staff put a cap back over the outlet.
Standard #: 22VAC40-185-350-B Description: Based on observation and interview the center did not ensure that the appropriate staff-to-children ratios were maintained.
Evidence:
1. There were eight 2-5 year old children in care with 1 staff member where 8:1 was required.
2. There was a volunteer in the class who could not be used in ratio.
3. Staff confirmed that the staff to child ratio was not met during the inspection.Plan of Correction: To continue to look for qualified staff that want to work so that we can continue to provide a needed service to our community.
