An unannounced complaint investigation was conducted this date. Staff Interviews were conducted, documentation reviewed, and classroom observations were conducted in response to a complaint alleging supervision and care and protection. The preponderance of evidence does support the allegations and the complaint has been deemed valid. Please email me at Karen.dickens@doe.virginia.gov with any questions or clarifications.

Standard #: 8VAC20-780-340-A Complaint related: Yes Description: Based on staff interviews, center documentation, and observation, staff did not ensure the care, protection and guidance of a child.
Evidence:
1. According to an Incident Report, on 05/02/2022, Child #1 was playing near the pretend clothing shelf when the shelf fell on them and "bumped" her on the right side of their face above the eyebrow.
2. Staff #1(Date of hire-06/13/2019) stated that she was across the room cleaning up with her back turned when she heard Child #1 scream. Staff #1 washed Child #1's eye area with soap and water and applied ice. After naptime, Child #1's eye was swollen. The next day, Child #1's right eye was black underneath.
3. Staff #2 informed the parent that the wardrobe would be either removed or bolted down to avoid future incidents.
4. Seven days after the incident, 05/09/22, the wardrobe was still present in the classroom and not bolted down.Plan of Correction: The wardrobe has been bolted to the wall to ensure that the wardrobe will not fall.
Standard #: 8VAC20-780-340-F Complaint related: Yes Description: Based on staff interview, children under 10 years of age were not within actual sight and sound supervision of staff.
Evidence:
1. Staff #1 stated that they were across the room with their back turned when the wardrobe fell on Child #1.Plan of Correction: Management has addressed supervision with staff member. The staff member will be retrained, coached, and will be provided with modeling by management. When available, management will provide additional support for classroom.
