Technical Assistance:
Reviewed updated Child Day Center standard effective 10/13/2021, where to find DOE model forms (childcareva.com) as well as the renewal application and staff form. The renewal form can be emailed directly to the inspector as no fee is required at this time. Please submit the renewal form in the next 2 weeks.

Comments:
An unannounced monitoring visit was conducted on-site. All rooms were in ratio with direct care staff. Observed children transitioning back into the building, infant care, feeding and diapering infants, preparation for lunch and reading. Lunch served today was pizza, salad and fruit. Reviewed a sample of staff records, medication authorizations and care plans, children records and physical space. Annual updates for injury plans and all permits are current. Staff have first aid and CPR certification, daily health observation and the administration team has MAT. Emergency medication is on-site. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-130-E
Description: Based on observation and staff statement, a child under the age of two does not have an updated immunization record every 6 months.
Evidence:
Child #1 did not have an update since 7/2021.

Plan of Correction: The center administration contacted the parent to request an updated immunization record.

Standard #: 8VAC20-780-570-E
Description: Based on observation, prepared bottles were not dated.
Evidence:
Two bottles in Infant I were missing dates.

Plan of Correction: A reminder will be sent to parents and staff will be retrained on checking the bottles are dated when they arrive at the center.
