Comments:
A monitoring inspection was initiated and concluded on 05/31/2022. There were 9 children present, ranging in ages from 3 years to 5 years, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed.

The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.

Standard #: 8VAC20-780-270-A
Description: Based on observation of the playground area it was determined that not all areas were maintained in clean, safe, and operable condition. Evidence: 1.) The playground structure is missing a side panel on the platform of the structure, posing a fall risk. 2.) The grass portion of the playground area had not been recently mowed, the length of the grass was over shin length. This poses a risk of potential ticks, snakes, or mosquitos to be present.

Description: Based on observation it was determined that not all hazardous substances were kept locked. Evidence: Laundry soap and a cleaning agent were observed in an unlocked closet, accessible to children.

Description: Based on observation it was determined that not all hazardous substances were clearly labeled indicating their contents. Evidence: A large bottle, not labeled, filled with liquid was observed by the LI (licensing inspector). Upon further interviews it was determined that the contents of the bottle contained a cleaning agent.

Description: Based on observation of the playground structure it was determined that the facility failed to maintain the required amount of resilient surfacing in the fall zone areas of playground equipment. Evidence: The amount of mulch on the playground was sporadic with dirt exposed beneath the swings, slides, and climbing component of the playground structure.

Description: Based on observation and interview it was determined that a second staff person was not immediately available to assist the program leader when one or more children were present. Evidence: The program leader was observed caring for 9 children in the lower school classroom. The assistant to the program leader was absent during the inspection. The administrator was located in the upper school in a different area of the campus. Based on interviews it was determined that when staff need assistance they are to call or email the administrator or call or email the staff in the upstairs portion of the lower school.
