4-13-22 Arrived 9:45am- 12:50pm
An unannounced licensing renewal study was conducted today.
There were 85 children present with 15 staff. The children were playing in centers, playing outdoors, eating lunch, planting spinach and resting. All areas of the standards were reviewed for compliance. Information gathered during the inspection indicated noncompliance with applicable standards or laws and are noted on the violation notice.

Standard #: 22.1-289.035-B-4
Description: Based on the review of staff files, the facility failed to ensure that all required background checks are complete for all staff.
Evidence:
1. Staff #3 indicated having lived in another state within the last five years on the sworn statement form.
2. There is no out of state criminal record report on file for staff #3.

Standard #: 22VAC40-185-140-A
Description: Based on the review of eight children's files the facility failed to ensure that each child has a physical health exam on file within 30 days of enrollment.
Evidence:
1. There are no physical health exams on file for child #3 & 4.

Standard #: 22VAC40-185-270-A
Description: Based on a tour of the playground, the facility failed to ensure that all equipment is maintained in safe and operable condition.
Evidence:
1. On the large playground, at the top pf the yellow slide, there are gaps of 1/4 inch between the support posts and the slide, presenting entanglement hazards.
2. Just above the yellow slide, on the upper deck, the plastic has pulled away from the deck, creating a tripping hazard.
3. On the small playground, the top of the tree house is not securely attached to the bottom section.

Plan of Correction: 1. Maintenance team is working to fix the slide gaps.
2. Maintenance has fixed mulch to cover areas of plastic and cut away the hazardous parts.
3. Maintenance has swept away mulch from tree house bottom section to refasten tree house properly.

Standard #: 22VAC40-185-280-B
Description: Based on a tour of the building, the facility failed to keep hazardous substances in a locked area.
Evidence:
1. The cleaning supplies cabinet over the sink in the Pre-K class was not locked.
2. The lock on the cleaning supplies cabinet in the toddler room is broken.

Description: Based on a tour of the playground, the facility failed to ensure that all ground supports are covered with materials that protect children from injury.
Evidence:
1. At the base of the yellow slide, there is a concrete footer exposed.

Plan of Correction: Maintenance has fixed mulch to cover concrete areas.
