Technical Assistance:
As discussed, a sample allergy action plan will be forwarded as a resource.

As a reminder, any new staff hired will need to complete the 10 Preservice training within 90 days of hire. Please maintain the certificates of completion at the program.

Comments:
An unannounced Monitoring inspection was conducted today. A total of 42 children with 3 staff were present. The children were finishing snack at the beginning of the inspection. They were observed sitting in small groups and had a choice of graham crackers, yogurt or apples with milk.
Children lined up to use the bathroom while staff positioned themselves at the door of the cafeteria to ensure adequate supervision. Tables were sanitized after snack time and the children transitioned to table top activities such as coloring with markers, playing chess or legos and using their laptops. Some medication onsite and reviewed. First Aid kits complete and staff First Aid/CPR, MAT and Daily Health Observation training was verified. Staff records are reviewed annually at the administrative office. A total of 6 children's records were reviewed and found complete. This inspection occurred approximately between 2:30pm and 4:15pm. Standard #: 8VAC20-780-40-M
Description: Based on review of documentation maintained at the center and one staff statement, a current written list of children's allergies was not found.
Evidence:
Staff #1 stated that the list of allergies posted was not current. Currently enrolled children with allergies were not reflected on list of children with allergies.

Plan of Correction: We will update the list of children with allergies as soon as possible.

Standard #: 8VAC20-780-60-A-8
Description: Based on review of medication, a written care plan for each child with a diagnosed food allergy was not found.
Evidence:
Child #5 and #6 have diagnosed food allergies and require emergency medication. A written care plan/allergy action plan was not on file for each child.

Plan of Correction: We will have the allergy action plans completed for both children.

Standard #: 8VAC20-780-510-E
Description: Based on review of medications onsite, the center failed to follow their own procedures for administering medication.
Evidence:
A long term emergency medication for Child #5 was onsite. A medication authorization signed by a physician was not maintained.

Plan of Correction: We will request a new authorization.

Standard #: 8VAC20-780-510-G
Description: Based on review of medications onsite, medication was not properly labeled.
Evidence:
1. The dosage amount and time/times to be given for Acetaminophen was not found for Child #6.
2. Zyrtec and Acetaminophen was not labeled with the name of Child #6.

Plan of Correction: We will request a new authorization from the doctor.
