Comments:
An unannounced inspection was conducted on 5/27/2022. There were 13 children present with two staff. The children ranged is ages 2 years to 10 years. Upon arrival to the center the children were preparing for snack. Hand washing and table sanitizing was also observed. Following snack the children went outside to the playground.
Five children's records and one staff record reviewed.
Inspection began 9:14 am ended 11:21 am.

Standard #: 8VAC20-780-190-A-4
Description: Based on observation and review of staff records the center failed to have a staff person present that meets program director qualifications.
Evidence:
Staff #1 hire date 11/1/13 and staff #2 hire date 9/3/10 do not have an educational background that meets the requirements for program director.

Plan of Correction: The owner/director will ensure that their is a program director present at least 50% of the time.

Description: Based on review of records the center failed to provide the licensing representative an annual fire inspection report.
Evidence:
The most recent fire inspection report provided by the center is dated 11/9/2020.

Plan of Correction: The fire marshal inspected the facility on 6/7/22.

Standard #: 8VAC20-780-260-B
Description: Based on review of records the center did not provide an annual approval from the health department.
Evidence:
The most recent health department approval is dated 4/7/21.

Plan of Correction: Will ensure that inspections are conducted annually.

Standard #: 8VAC20-780-270-A
Description: Based on observation the center failed to ensure that the areas and equipment of the center, outside, are maintained in a safe condition.
Evidence:
The ground covering under the slide on the playground is torn and is a tripping hazard.

Plan of Correction: The staff will make sure the playground is safe.

Standard #: 8VAC20-780-350-B-3
Description: Based on observation the center failed to maintain the one to eight ratio for the two year old ages.
Evidence:
During the inspection on 5/27/2022 13 children ranging in age from 2 years-10 years went outside to the playground. One staff person went outside with the 13 children.

Plan of Correction: The staff will make sure ratios are met at all times.
