Comments:
The Licensing Inspector conducted an unannounced monitoring inspection on Thursday June 9, 2022 from approximately 9:00 am to 11:00 am. There were 68 children in attendance and a total of 13 staff present. 5 staff records and 5 children?s records were reviewed. All areas of the center including classrooms, hallways, kitchen, bathrooms, and the playgrounds were observed and inspected. Infant bottles were labeled and dated correctly. The children and staff were engaged in a variety of activities while the licensing inspector was on the premises. Children were observed playing on the playgrounds, getting ready for water play, reading, singing songs, dancing, having their diapers changed, and learning about insects. Emergency drills and inspections were in compliance. Health Inspection - 10/13/21; Fire Inspection- 04/07/22; evacuation drills conducted monthly-most recent on 05/31/22; shelter in place drill conducted twice-most recent 03/31/22; lock down drill conducted annually. The first aid kit had all components. The license, current inspection, evacuation plan, and menu were posted, and emergency numbers were posted by each phone.

Violations were cited as a result of this inspection and are identified on the violation notice.

Standard #: 8VAC20-780-150-B
Description: Based on records review, the center did not ensure that the original physical examination was signed and dated by a physician. Evidence: Child #2 had a physical that was not signed or dated by a physician.

Plan of Correction: Before start date will confirm signature/stamp and date from doctor is on health forms. The updated form was in the file with the signature and date of a physician.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence:1) Classroom #6 contained an approximately 8 inch by 2 inch area of exposed drywall and peeling paint. 2) Classroom #6 had a broken counter edge that was sharp to the touch.

Plan of Correction: Work orders have already been placed. Will walk through with maintenance crew to correct paint and counter.

Standard #: 8VAC20-780-280-B
Description: Based on observation the center did not ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1) Classroom #7 had an unlocked cabinet which contained 2 cans of Barbasol shaving cream with labels that read ?keep out of reach of children? and ?warning?. 2) The Kitchen door was unlocked and had a piece of tape over the door latch that prevented it from closing. The kitchen contained unlocked cleaning agents.

Plan of Correction: Corrected on site during visit.
