Comments:
An in-person monitoring inspection was conducted on June 6 and June 8, 2022. There were 12 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: Based on record review, the center did not complete repeat background checks every five years.

Evidence: Staff 2 has a central registry background check which was last conducted on June 1, 2017.

Plan of Correction: A repeat CPS check will be sent out for the provider on Monday, June 13, 2022.

Standard #: 22.1-289.035-B-2
Description: Based on record review and interview, all staff did not submit to fingerprinting.

Evidence: Staff 1 did not complete a fingerprint based criminal history record check.

Plan of Correction: The staff member will not be returning as a substitute,

Standard #: 8VAC20-770-60-B
Description: Based on record review and interview, all staff did not have a completed sworn statement or affirmation.

Evidence: Staff 1 did not have a completed sworn statement or affirmation by their first date of employment. (Staff 1, DOH 6/6/2022)

Plan of Correction: The staff member will not be returning as a substitute.
