Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation and interview on June 7, 2022, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: 1. In the pre-school classroom, the bottom of the sink was observed to be broken creating sharp edges and there were protruding nails that could snag children's clothing or skin.
2. Bus #1 had a hole in the seat approximately an inch in circumference which exposed the inner foam material.
3. Bus #3 had a hole approximately 10x5 which exposed the inner foam and seat material. The foam was ripped and missing in several areas.
4. Administration acknowledged the broken sink and the holes in the bus seats.

Plan of Correction: Sink will receive repair, will immediately patch holes in bus seats and take to seek repairs.
