Technical assistance was offered related to: Background checks, FARE allergy action forms, new center policies, lockdown drills, new staff orientation.

An unannounced monitoring inspection was conducted today from 10:50 am to 12:15pm. There were 71 children in direct care and supervision of 9 staff members. The center has three classrooms that maintain mixed age groups of 3-5 year old children. The children were observed engaged in circle time, small group learning activities, playing outdoors and lunch. Lunch from home was labeled.
3 children's records, 3 staff records, medications, center's policies, emergency drill records and the emergency plan were reviewed. Areas of concerns are listed in the violation notice. Standard #: 22.1-289.035-B-4 Description: Based on records review, the center did not obtain results of an out of state criminal record check (CRC) for an employee prior to the first day of employment; and the center did not obtain results of an out of state central registry search for an employee within 45 days of employment.
Evidence:
1. Staff # 1's first day of employment was 12/20/21. Staff # 1's record indicated that she had resided in Illinois within the last 5 years.
2. Staff # 1's CRC for Illinois was dated 1/13/22, and was not obtained prior to the first day of employment.
3. Staff # 1's record was missing the results of an out of state central registry search for Illinois.
3.Plan of Correction: Will contact agencies and get proper documentation out.
Standard #: 8VAC20-770-60-C-2 Description: Based on records review and staff interview, the center did not obtain results of the Virginia central registry search for an employee within 30 days of employment.
Evidence:
1. Staff # 2's first day of employment was 4/25/22.
2. Staff # 2's central registry form that she had completed was still in her record, and had not been mailed to the appropriate agency.Plan of Correction: The form will be mailed out today.
