An unannounced complaint inspection was initiated on 5/25/2022 in response to a complaint that was received by the licensing department on 5/24/2022 regarding allegations of inadequate supervision, building maintenance and noncompliance in the areas of diapering and injury reporting. The inspection was initiated on 5/25/2022 and completed on 06/13/2022. On 05/25/2022, the inspector was on site from 9:40 am to 11:08 am. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. Additional interviews were conducted remotely.
The preponderance of the evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.

Evidence:
1. One injury report reviewed noted that a minor injury occurred on 12/28/2020 and the parent was notified on 12/30/2020.

Standard #: 8VAC20-780-550-P Complaint related: No Description: Based on a review of 46 injury reports and interview, the center did not ensure that 1 report contained the required information. The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the following: 1. Date and time of injury; 2. Name of injured child;
3. Type and circumstance of the injury; 4. Staff present and treatment; 5. Date and time when parents were notified; 6. Any future action to prevent recurrence of the injury; 7. Staff and parent signatures or two staff signatures; and 8. Documentation on how parent was notified.
Evidence:
1. One injury record reviewed was dated 12/23/2021 and noted that a child had sustained a minor injury. The injury report was missing the date and time of parent notification.
