Technical Assistance:
Inspector reminded reminded site supervisor that a second shelter-in-place drill and a June fire drill is required to be practiced before the end of the school year.

The next anticipated inspection will be the providers renewal inspection. Provider was reminded to have all renewal application documents completed and sent into licensing office 45 days prior to expiration for processing. The renewal application is available on the VDOE website.

Comments:
Today an unannounced Monitoring Inspection took place between the hours of 3:00pm and 4:30pm. There were 2 rooms observed with a total of 48 children with 6 staff within the supervision guidelines. The children were observed eating snack, playing with blocks, doing art, playing in the school gym and being picked up by their parents. A complete inspection of the physical plant, children, fire drill log, medication and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review, each child's record does not contain a physical exam.
Evidence: Child #4's record does not contain a physical exam.

Plan of Correction: I'll have to talk to the office about reaching out to the parents to get the physical.

Standard #: 8VAC20-780-40-M
Description: Based on observation, the center did not maintain a current written list of all children's allergies, sensitivities and dietary restrictions.
Evidence: Child #7 had a diagnosed food allergy and emergency medication is kept on site. Child #7's name and allergy was not listed on the programs allergy list.

Plan of Correction: I'm working on a new list that is easier to read. I'll make sure everyone's names are on the list.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method.
Evidence: Several spray cleaning agents were observed sitting inside of a Tupperware contained on a desk in the cafeteria.

Plan of Correction: Corrected during the inspection.

Standard #: 8VAC20-780-510-P
Description: Based on a review of medication, medication was not returned or discarded within 14 days after an authorization expired.
Evidence: Two out of three medication authorizations for Child #6 expired on 5/6/2022. Both medications were observed on site during today's inspection.

Plan of Correction: I can speak to the parents about filling out a new medication form.

Standard #: 8VAC20-780-560-F
Description: Based on observation and interview, requirements were not met when the center chose to provide snacks.
Evidence: A current food menu for the month of June is not posted. The children were observed eating a snack served by the program. The site supervisor stated that the June menu was not emailed to her.

Plan of Correction: I'll ask to get the June menu emailed to me to post.
