Comments:
An unannounced complaint investigation was conducted on 6/8/22 in response to a complaint received in the licensing office regarding allegations of forbidden action, rough handling; forbidden action, enclosure in a confined space; inappropriate behavior guidance; and unsanitary practices. Based on a review of documentation, interviews conducted, and classroom observation, the allegations were unable to be substantiated due to insufficient corroborating evidence. Violations were cited that were not part of the complaint but were found during the investigation. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 6/17/22. Description: Based on review of documentation, the center did not ensure that staff completed annually a minimum of 16 hours of training appropriate to the age of children in care. Evidence: Annual training was requested for Staff A. The certificates provided for Staff A added up to 3.75 hours of annual training and were dated from 2020 to 2021.

Plan of Correction: Teacher will complete her trainings.

Description: Based on review of documentation and interviews conducted, the center did not maintain a written record of children?s serious and minor injuries in which entries were made the day of occurrence and contained all required information. Evidence: Interviews revealed that on approximately four occasions over a one month time period, an infant bit or scratched another child in the infant classroom. The incident reports for the four incidents were requested, but only one incident report was provided to the Licensing Inspector.

Plan of Correction: Retrain staff on incident reports and documenting.
