Comments:
An investigation was initiated on 04/27/2022 with the local agency DSS to investigate a complaint involving parental notification and behavioral guidance. Interviews were conducted, documentation, child, and staff records were reviewed. The allegation involving parental notification and behavioral guidance is determined to be valid.

Description: Based on interviews it was determined that behavioral guidance was not always appropriate and constructive in nature. Evidence: On April 20, 2022 at about 8:30 a.m. It was reported that staff A used their arm to push child A against the wall. Child A was asked to put away their toys from home. When the child refused, staff A began yelling, approached the child and asked them to put the toys away again. The child then lunged at and attempted to strike staff A. Staff A then put their arm up to block child A from lunging at them and also took hold of child A's wrist to prevent the child from striking staff A. During this interaction it was reported that Staff A used their arm to push child A against the wall. Child A then put their toy away and moved to sit down at a table. It was reported that child A put their head down on the table until it was time to leave for school.

Plan of Correction: We will train on appropriate positive guidance. We will communicate with parents regarding behavior issues. We will document all behavior incidents moving forward. We will use our school cameras to better assess any future incidents. We will report any inappropriate guidance to CPS in the future as well.
