Technical Assistance:
- Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
- For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Comments:
A monitoring inspection was initiated on 6-16-22 and concluded on 6/16/22. There were 43 children present, ranging in ages from 10 months to 7 years, with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 4 staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-780-240-A
Description: Based on record review and interview, the center failed to ensure all staff completed the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence:

1. The file for staff 3 documented the start date as 1/28/22. The file did not contain documentation of staff 3 completing the Virginia Department of Education-sponsored orientation course.
2. The director verified the start date and stated staff 3 has not completed the course.

Plan of Correction: Staff 3 will be required to complete the Virginia Department of Education-sponsored orientation course by the end of the month. In the future all new staff will be required to complete the orientation course within 90 days of employment or they will not be allowed to return to work until completed.

Standard #: 8VAC20-780-350-B-2
Description: Based on observation and interview, the center failed to ensure the staff to child ratio of one staff to every five children for ages 16 months up to 24 months was followed.

Evidence:

In the two to three year old classroom one child was 21 months old per the director. There were 15 children with two staff. The remaining 14 children were all ages two and three.

Plan of Correction: The 21 month old child will be moved to the younger class until he turns two.
