Provided consultation on standard:
1) 40M: The center shall maintain a current, written list of all children's allergies, sensitivities, and dietary restrictions; the list is to be updated when children are enrolled/disenrolled
2) 270A: Areas of the center outside (playground) shall be maintained in a safe condition; ensure that the wood timber ties and weeds within reach of the perimeter of the fence are not splintered or sharp to touch
3) 500B: The diapering area shall have a leakproof covered receptacle for soiled linens; the diapering surface shall be used only for diapering children

Comments:
An unannounced monitoring inspection was conducted from 10:15 am ? 1:15 pm with the director and staff. There were 119 children, ranging from 3 months to 10 years of age, present with 20 staff supervising. Children were observed playing in centers, participating in circle time activities, and playing outside on the playground. Five children's records and five staff records were reviewed. Two emergency medications, authorization forms and allergy care plans were reviewed and in compliance. The emergency drill log, daily attendance, written allergy list, menu, and fire and health inspections were reviewed.

Standard #: 8VAC20-780-140-A
Description: Based on a review of five children?s records, the center did not obtain documentation that each child has received a physical examination prior to the child's first attendance or within one month after attendance or within 90 days grace period for a documented homeless child. Evidence: the record for child #3 (date of first attendance: 10/7/2021) did not have documentation of a physical examination.

Standard #: 8VAC20-780-160-A
Description: Based on review of five staff records, the center did not have documentation of a tuberculosis (TB) test/ screening for staff at the time of employment and prior to coming into contact with children. Evidence: the record for staff #2 (date of hire: 3/14/2022), staff #3 (date of hire: 6/13/2022 ) and staff #4 (date of hire: 5/16/2022) did not have documentation of a TB test/screening.

Description: Based on observation, cleaning and sanitizing materials were located above food utensils and single-service food articles. Evidence: in the school age classroom and three?s classroom bottles of cleaning and sanitizing solutions were observed on a shelf above disposable cups, plates and plastic eating utensils.

Description: Based on observation, the facility failed to ensure that staff wash their hands with soap and water as required. Evidence: staff in the two's classroom were observed changing a child's diaper and then updating the tablet before washing their hands.

Description: Based on observation, the facility failed to ensure that diapers are disposed of in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: staff in the two?s classroom were observed disposing of a diaper using their hand to open the trash can lid and not their foot.

Description: Based on observation, the center did not have a current menu listing the foods to be served as required. Evidence: the menu that was posted in the kitchen was dated: 6/22/2020, was not posted in a conspicuous place or provided to parents, per staff, and there was no menu from the previous week kept on file.
