An investigation to review a self-reported incident regarding supervision was conducted on April 29, 2022 beginning at 11:30 a.m. Staff interviews were conducted. Documentation was reviewed.
Information gathered during the inspection determined non-compliances with applicable standards or laws and a violation was documented on the violation notice issued to the program.

Standard #: 8VAC20-780-340-F Description: Based on staff interviews and review of documentation, it was determined that not all children under the age of 10 years old were within actual sight and sound supervision of staff. Evidence: On April 22, 2022, in the two year old classroom, Child A, age two, was left alone in the classroom from approximately 3:30 p.m. to 3:34 p.m. Plan of Correction: Not available online.
