Comments:
A monitoring inspection was conducted on June 17, 2022 and concluded on June 17, 2022. On June 17, 2022, the licensing inspector was onsite from approximately 10:30am until 1:30pm. The director assisted with the inspection. There were 58 children present, ranging in ages from five months to seven years, with ten staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and eight staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on review of eight staff records and interview, the center did not request an out of state abuse and neglect search for each state an individual has resided in the past five years by the end of the 30th day of employment for one staff.

Evidence:

1. The record of staff #1, hired 04/25/2022, indicated staff #1 had resided in another state in the past five year. The record did not contain documentation of a abuse and neglect check request made within 30 days of employment.
2. The director stated an out of state abuse and neglect check had not been requested for staff #1.

Plan of Correction: Per the director: I missed the fact she had lived out side of Virginia and did not request an out of state background check. I will send off the request today.

Standard #: 8VAC20-780-160-A
Description: Based on review of eight staff records, the center did not obtain documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children that had been completed within the last 30 days prior to employment for four staff records.

Evidence:

1. The record of staff #1, hired 04/25/2022, had documentation of a negative tuberculosis screening dated 04/29/2022, exceeding the date of employment.
2. The record of staff #6, hired 04/15/2022, had documentation of a negative tuberculosis screening dated 06/12/2022, exceeding the date of employment.
3. The record of staff #7, hired 06/03/2022, had documentation of a negative tuberculosis screening dated 02/11/2022, exceeding 30 days prior to employment.
4. The record of staff #8, hired 06/17/2022, did not have documentation of a negative tuberculosis screening at the time of employment or prior to contact with children. Staff #8 was observed working in the three year old classroom.

Plan of Correction: Per the director: I will have all staff obtain a TB screening prior to hire that is no more than 30 days old. I will send staff #8 to get her TB screening completed today.

Standard #: 8VAC20-780-240-A
Description: Based on review of eight staff records and interview, the center did not ensure two staff completed the Virginia Department of Education sponsored orientation course (VA preservice) within 90 calendar days of employment.

Evidence:

1. The record of staff #3, hired 03/07/2022, did not contain documentation of VA preservice training exceeding 90 calendar days from employment.
2. The record of staff #5, hired 01/24/2022, did not contain documentation of VA preservice training exceeding 90 calendar days from employment.
3. The director stated the two staff had not completed the required training.

Plan of Correction: Per the director: I have the staff working on completing the VA preservice training now. I will check to see they all complete the training by the end of the month and new staff complete it within 90 days of hire.

Standard #: 8VAC20-780-510-G
Description: Based on review of medication, the center did not ensure medications were labeled with the child?s name, the name of the medication, the dosage amount, and the time or times to be given.

Evidence:

The prescription medication for child #2 was not in its original box nor did it have the pharmacy label with the dosage, time or times to be given.

Plan of Correction: per the director: I will contact the parent and request that they provide the original box and label for the medication.

Standard #: 8VAC20-780-530-A
Description: Based on review of staff records and interview, the center did not ensure at least one staff in each classroom or area where children are present current certification in cardiopulmonary resuscitation and certification in first aid training.

Evidence:

1. The record of staff #7, the only staff assigned and present in the school age classroom, did not have documentation of current CPR and first aid certification.
2. The director stated staff #9, the only staff assigned and present in the toddler classroom, did not have current CPR and first aid certification.
3. The director stated staff #10, the only staff assigned and present in the pre-k classroom, did not have current CPR and first aid certification.

Plan of Correction: Per the director: I have scheduled a class for all staff for next Thursday. Staff in each classroom will have current certification in CPR and first aid.
