Comments:
An unannounced monitoring inspection was conducted today between the hours of 12:00pm-1:015pm. There were a total of 13 children with four supervising staff members present. Children were observed during naptime, engaged in individual play and handwashing. A selection of staff and children records, and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1. In the infant classroom, the wood on the edge of the diapering table is cracked and missing a three inch piece of wood. This area can entangle clothing and snag skin.

Plan of Correction: A work order has been submitted to replace the cracked wood.
