An unannounced renewal inspection was initiated and concluded on 6/15/2022. The inspection began at 9:30 am and concluded at 12:18 pm. There were 72 children present, ranging in ages from 5 months to 5 years, with 17 staff supervising. The center's director assisted the inspector throughout the inspection. During the tour, the inspector observed the children participating in circle time, tummy time, playing with puzzles, free play, and interacting with peers and staff. The inspector observed all 8 classrooms, playgrounds, kitchen and restrooms. The classrooms are equipped with age and stage appropriate materials. The center's first aid kit and non-medical emergency supplies were inspected and found complete. Liability insurance, health inspection, and drills were all in compliance. A total of 7 child records, 7 staff records, and 3 board member/agent records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-2 Description: Based on a review of 7 staff records and interview, the center did not ensure to obtain
the results of a fingerprint background check prior to the first date of employment for 1 staff as
required. Evidence:
1. The record of staff #7 (DOH: 8/16/2021) contained a fingerprint background check was dated
8/20/2021.
2. Administration acknowledged that the fingerprint background check was received after the first date

Standard #: 8VAC20-780-160-A Description: Based on a review of 7 staff records and interview, the center did not ensure that documentation of a negative tuberculosis screening was obtained for 1 staff at the time of employment and prior to coming into contact with children. Evidence: 1. The record of staff # 6 (DOH: 5/16/2022) contained a TB screening dated 5/17/2022. 2.
