Comments:
A complaint investigation was initiated on April 13, 2022, regarding a three year old child medication was not administered when child had an allegoric reaction and child in soiled clothing. Interviews were conducted. Based on interviews conducted and documentation reviewed, there was not a preponderance of evidence to support the allegation regarding medication and toileting procedures; however, the complaint was deemed valid for children records identified in the Violation Notice.

Description: Based on children records review, the center did not maintain children records and reports were accessible for two years after termination of services.
Evidence:
1. Staff #1 was not able to provide Child #1's record to review documents during the time child was enrolled.
2.Staff #1 administered medication to Child #1. Staff #1 was not able to provide medication record to review dates medication was administered.
3. Child #1 is no longer enrolled at the facility.
