Comments:
A renewal inspection was conducted on June 17, 2022 approximately 11:25 am to 1:10 pm. There were 69 children present directly supervised by 613staff. The children were observed in resting, eating lunch, hand washing, and circle activities.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. The inspector reviewed on 5 child?s record and 13 staff records.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-185-160-C
Description: Based on staff records review, the center did not maintain at least every two years rom the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members obtain and submit the results of a follow-up tuberculosis screening.
Evidence
1. Staff #7 tuberculosis screening expired on 4/16/2021.

Standard #: 22VAC40-185-270-A
Description: Based on classroom observation, inside equipment was not maintained in safe and operable condition.

Evidence:
1.In the school-age classroom, a table with a black covering had several tears on top of the table.
2. In the toddlers classroom, a seating chair had a large visible tear in the seat.
3. In the two?s classroom, 8 visible napping mats had visible rips in the corners of the mat.
