Technical Assistance:
Discussed the additional training and preservice orientation training that is now required for all new staff hired since
October 13, 2021. A First Aid/CPR training curriculum will be forwarded to use as a resource. A staff training log will also be
forwarded to assist with documenting annual staff training.

Comments:
A self- report was received regarding care and protection of one child. An onsite unannounced inspection was conducted.
Staff interviews were conducted, staff written statements reviewed and observation of classrooms was made. Collateral
Contacts were interviewed via telephone on 05/24/2022. The preponderance of evidence does not support a valid finding
with regards to care and protection, however one violation found regarding mandated reporter requirements.
A monitoring inspection also took place this date. Observation of all classrooms and outdoor play areas was made. Both
child and staff records were reviewed. Some violations were found during this inspection. Thank you for your cooperation

Standard #: 8VAC20-780-80-B
Description: Based on staff interviews and staff written statements, a report of a suspected incident of child abuse was not reported in accordance with ? 63.2-1509 of the Code of Virginia.

Evidence:
Staff #1 suspected physical abuse involving Child #1. Staff #1 failed to report the suspected incident of child abuse to the local government agency and/or Center Administration.

with the entire staff on May 26th at our

Standard #: 8VAC20-780-180-A
Description: Based on staff interviews and documentation review, staff failed to carry out their assigned responsibilities in accordance with the Center's required training.

Evidence: Despite documentation of completed required training, Staff #1 was unfamiliar with the responsibilities of a mandated reporter and stated that ?this is the first time I?ve heard of that term mandated reporter?. Staff #1 acknowledged their signature on the training documentation dated 08/06/2021.

Plan of Correction: Review what is means to be a

should be made. This was addressed
with the entire staff on May 26th at our
staff meeting. All staff will sign saying
they have received this training.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation of the Center, protective covers for electrical outlets were not found.
Evidence:
The electrical strip located in the before and after school classroom had 4 outlets that did not have protective coverings. This classroom was currently being used by preschool children.

Plan of Correction: Ensure that all electrical outlets and
electrical strips have protective
coverings.

Standard #: 8VAC20-780-350-B-3
Description: Based on observation of the Butterfly classroom, a ratio of a 1:8 was not maintained.
Evidence:
The Butterfly class had a total of 14 children (2 years of age) with 1 staff. The second staff had left the classroom to use the bathroom.

Plan of Correction: Reviewed all ratio requirements with
staff including:
-Counting children during transitions
-Teacher bathroom breaks
-Sight and sound requirements
-Children bathroom breaks
-Informing administration when a
teacher is has reached maximum
ratio.
This was reviewed with all staff on
5/26/2022 and again on 6/13/2022

Standard #: 8VAC20-780-470-A
Description: Based on observation of the outside area, an outside pool was not locked when not in use.
Evidence:
1. The gate leading to the outside pool was left open and unlocked by a maintenance contractor.
2. Children were outside playing and riding tricycles adjacent to the open gate.
3. Two staff were present outside during this time and failed to close the gate leading to the pool.

Plan of Correction: Discussion with our facilities manager
to ensure he knows that gate needs to
be always locked or a staff member
needs to be stationed at the gate to
ensure no children enter. This
includes anytime that we have pool
maintenance occurring.

Standard #: 8VAC20-780-560-L
Description: Based on observation of lunch time, staff failed to sit with children while they ate.
Evidence:
Some children were observed finishing their lunches while staff were observed sweeping food that had dropped onto the floor and setting up cots in order to transition to nap time.

Plan of Correction: Reviewed teacher?s roles during
mealtimes and let them know that all
teachers must be seated with children.
This was reviewed at our staff meeting
on 5/26/2022/
