Comments:
A monitoring inspection was initiated on 6/21/22 and concluded on 6/30/22. There were 48 children present, ranging in ages from 5 to 10, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 3 staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to keep hazardous substances in a locked place using a safe locking method that prevents access by children.

Evidence:

In the back building, which contains the single classroom used by children, a spray bottle of Cloralen bathroom cleaner with bleach was on a table in the classroom. The storage closet in the classroom was left unlocked. The closet contained cleaners such as Cloralen and Spray Disinfectant.

Plan of Correction: All hazardous substances will be moved to a locked location and staff will be trained on this standard. The director will complete spot checks in the classrooms to ensure staff are following the requirement.

Standard #: 22VAC40-185-540-E
Description: Based on observation and interview, the center failed to have the required nonmedical emergency supplies.

Evidence:

1. There was not a working battery operating flashlight in the older children's classroom.
2. The staff in the older children's classroom did not know where the battery operating radio was.
3. The older children's classroom is the only class in that building.

Plan of Correction: The batteries will be replaced in the flashlight. On the first of each month the flashlight will be checked to make sure it is still working. All staff will be reminded of the location of the radio.

Standard #: 8VAC20-820-120-E-2
Description: Based on observation, the center failed to post the findings of the most recent inspection notice in a location parents can observe.

Evidence:

1. Both classrooms and the entrance of the center were observed. The inspection notice was not found.
2. The director stated the inspection notice is not posted.

Plan of Correction: When the results of this inspection are received they will be posted in a location parents will see.
