An unannounced monitoring inspection was conducted on June 15, 2022. At the time of entrance there were 40 children and 10 staff members present with children. The sample size consisted of 3 children's records and 3 staff records. Children were observed eating lunch , preparing for nap and napping. The inperson inspection began at approximately 11:37 am and ended at approximately 1:47 pm. Areas

Standard #: 8VAC20-780-160-A Description: Based on record review, licensee did not ensure that the documentation of a negative tuburculosis screening should be completed within the last 30 calendar days of the date of employment.
Evidence:
Staff #3 hire date was 06/10/2022 the tuburculosis test results were provided was from November 5, 2021, which is more than 30 calendare days of the date of employment.Plan of Correction: Have staff person complete TB screening week of 06/27/2023.
Standard #: 8VAC20-780-40-M Description: Based on observation in classrooms, the licensee did not ensure the list is dated.
Evidence:
In the Pre-K classroom, the velcro allergy list did not have a date on the child with the allergy.Plan of Correction: Put dates on all allergy list today.
Standard #: 8VAC20-780-270-A Description: Based on observation, the licensee did not ensure outside was maintained in clean , safe and operable condition.
Evidence:
On the infant/toddler playground, there was a cracked plastic container that could snag skin or pinch a child. The climbing hand and foot pieces on the playground composite on both sides are cracked which has sharp points.Plan of Correction: Notify facilities to put in a work order immediatley to remove climbing hand and foot pieces.
Standard #: 8VAC20-780-280-B Description: Based on observation and staff interview, the licensee did not ensure hazordous substances such as cleaning materials shall be kept in a locked place using a safe locking method.
Evidence :
In the infant room, the soap and water and bleach and water spray bottles were in basket next to diapering area and preparation are for infant food. THere was only one infant in classroom and diapering was not taken place at the time.
In the toddler classroom, the soap the soap and water and bleach and water spray bottles were in basket next to diapering area and children were not being diapered at the time of observation.Plan of Correction: Remind staff to put hazards substances away locked when not in use.
