Technical Assistance:
Discussed background check requirements, staff records, children's records, medication requirements, diaper ointments/over the counter topical skin products.

Comments:
An unannounced monitoring inspection was conducted beginning at 9:15 a.m. and ending at 12:00 p.m. with the director. There were 48 children in care ages 5 months through 5 years old under the care and supervision of 10 staff. Children were observed engaged in a variety of activities including centers for drawing, pretend play, and blocks. Infants were observed having play time on the floor and sleeping. Toddlers were observed during outdoor play. Classrooms and materials were observed. The outdoor play areas and playgrounds were observed.
Six staff records, six children's records and five medications and authorizations were reviewed.

Information gathered during the inspection determined non-compliances with applicable standards or laws and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-A
Description: Based on review of documentation, it was determined that the center failed to obtain Central registry background checks every five years. Evidence: Staff #3?s central registry background check results were dated 9/29/2016.

Standard #: 8VAC20-780-160-A
Description: Based on review of staff records, the center did not obtain tuberculosis (TB) screenings or test results before staff came in contact with children or TB documentation that was completed no later than 30 days before employment. Evidence: Staff #4, start date 4/19/2022, did not have documentation of a TB in the file. Staff #5, start date 1/11/2022, TB documentation was dated 9/22/2021.

Standard #: 8VAC20-780-245-J-3
Description: Based on review of documentation and interview with staff, the center failed to ensure staff were properly trained in emergency medication administration for rectal medications. Evidence: Child #3, has prescription diazepam on site, that is required to be administered rectally and no staff on site have the required Medication Administration Training (MAT) Rectal Medication Certification.

Description: Based on review of documentation and interview with staff, it was determined that the center did not follow it's medication policy that includes obtaining parent authorization for all medication for children in care. Evidence: Child #3, has nonprescription Tylenol on site and there was no signed authorization from the parent.

Description: Based on observation, the center failed to ensure medication requirements were followed. Evidence: In the Two?s classroom/Preschool Explorers, a tube of prescription triamcinolone acetonide was observed in an unlocked plastic cubby above the diapering area. The prescription medication did not have the original medication label, was not labeled with the child's name, the dosage amount, and the time or times to be given.
