An unannounced monitoring inspection was conducted beginning at 9:00 a.m. and ending at 10:30 a.m. with the director. There were 11 children in care ages 4 months through 4 years of age under the care of 4 staff. Three staff records and four children's records were reviewed. One medication and authorization were reviewed.
Information gathered during the inspection determined non-compliances with applicable standards or laws and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-4 Description: Based on review of documentation and interview with staff, it was determined that the center did not request an out of state central registry background check for staff that resided outside of the state of Virginia in the last five years as required. Evidence: Staff #1, start date 3/21/2022, indicated out of state residence on the sworn statement of affirmation in New York in the last five years and there was no out of state central registry background request on file for New York.
