Technical Assistance:
Discussed regulations for documentaiton for program leaders and program director ( resume with programatic experience and management experience), MAT staff on duty, handsanitizer and labeling of soap bottles, crib spacing when occupied, new orientaiton requirements and updated model forms, allergy , dietary restrictions and sensitivity list acessible for staff with date, and outside play area that is not in use.

Discussed 22.1-289.058. Child day programs -Each building that was built before 2015 and that houses a child day program that is licensed that serves preschool-age children shall be equipped with at least one carbon monoxide detector.

Comments:
An unannounced monitoring inspection was conducted on June 14, 2022. At the time of entrance there were 67 children and 10 staff members and additional administrative staff available.The sample size consisted of 4 children's records and 4 staff records. Children were observed eating breakfast, preparing for circle time, a fitness activiity indorrs for toddlers, art activiites and outside. Records were reviewed by email. Areas of non-compliance are identified on the violation notice.

Description: Based on staff interview and record review, the licensee did not ensure there was at least one staff member on duty who has obtained with in the last three years instruction in performing the daily health observation of children.

Evidence: There was not a staff member on duty with Daily Health Observation during the inspection. Staff #5 confirmed she was unable to locate a record of a staff member with Daily Health Observation.

Description: Based on observation and interviews, the center did not ensure there was resilient surfacing underneath all fall zone areas of play equipment that meet minimum safety standards described in the American Society for Testing and Materials.A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. A fall zone need to be at least 6 feet surounding the equipment that requires resilent surfacing and the fall zone need to be free of hazard obstacles.

Evidence:

1. On the playground composite with yellow slide, the wooden swings, and red climbing arch there 0-1inch of resilent surfacing around the fall zones. The mulch has turned into soil.

2. On the playground with the ship , the peag gravel around the ship with slide and swings in multiple place had 4 inches of resilent surfacing.

3. The playground area under the pavillion next to ramp, there was moveable slide equimpment and climbing equipment that children were observed playing on but did not have 6 feet fallzone.

2. On the playground with pea gravel, the swing area had 4 inches of pea gravel.

Description: Based on observation and staff interview, the licensee did not ensure the diapering surface was non-absorbant and the diapering pad was cleaned with soap and water and sanitize.

1- Diapering pad in the toddler room had a tear which made the surface absorbant.
2- In the 2 year old classroom, the diapering surface was not cleaned with soap and water. Confirmed with staff.
