Technical Assistance:
Discussed with Director and Owner: Age range for children based on the Certificate of Occupancy.

Comments:
An in person monitoring inspection was conducted today. All rooms are in ratio during the inspection. Children were observed resting and playing on the playground during the inspection. The menu, rest cot spacing, top and bottom covers for rest cots, emergency evacuation drills, shelter-in-place and lockdown drills, and training hours re all in compliance.
2 staff and 3 child records were reviewed.

Violations:
Standard #: 8VAC20-780-160-C
Description: Based on a review of records, one staff record does not contain documentation of the follow- up results of a tuberculosis screening within 2 years from the previous results.
Evidence:
1 of 2 staff records contained a TB screening result dated 2/1/2020. (expired)

Plan of Correction: The specific member of the staff was asked to schedule an appointment with their doctor to have the physical forms and TB screening updated/renewed.

Standard #: 8VAC20-780-60-A
Description: Based on a review of records, one child record does not contain documentation of all required components.
Evidence:
1 of 3 records reviewed did not contain documentation of viewing Proof of Birth documents. (Child 1)

Plan of Correction: The student's parents were asked to bring the child's birth certificate so we can obtain proof of birth and complete the form.

Standard #: 8VAC20-780-510-I
Description: Based on a review of medications, one medication is not in the original box with the prescription attached.
Evidence:
One Epi-pen at the facility is not in the original container with the prescription label attached.

Plan of Correction: The need of getting the original box with the prescription label attached was discussed with the student's parents. they will talk to their doctor/pharmacist.

Description: Based on a review of first aid supplies, first aid kits one each floor are incomplete.
Evidence:
2 of 2 first aid kits inspected did not contain 2 triangular bandages each.

Plan of Correction: Buy triangular bandages and add them to our first aid kits.

Standard #: 8VAC20-780-550-C
Description: Based on a review of emergency evacuation maps, shelter-in-place locations are not denoted on the maps.
Evidence:
The shelter-in-place locations are not noted on the maps posted for the facility.

Plan of Correction: The maps of the school will be reviewed and re-done to add shelter-in-place locations. The changes will be posted in the office and the classrooms.
