Comments:
An onsite unannounced monitoring inspection was conducted on June 24th and concluded on June 28, 2022. There were 44 children in care ages one month through 12 years old. There were six staff providing direct care and one administrator. A walking of the entire facility inside and outside was completed. A sample of children and staff records were digitally reviewed. Children were observed in teacher and self-directed activities including but not limited to play activities, transitions, pencil and paper tasks, crafts, lunch and restroom routines. Required postings, annual inspections, emergency equipment and emergency evacuations were reviewed. Violations were observed and appear on the violations notices and were reviewed with center management during the exit interview.

Violations:
Standard #: 8VAC20-780-40-M
Description: Based on observation and interview the center failed to maintain a current written list of children's allergies in each room or area where children are present.
Evidence:
1 - There was no information regarding allergies and restrictions for child 1 available in the two year old classroom where the child was in care.
2 - Staff 2 stated that she was unaware of the allergy needs of child 1.
3 - The emergency medication for child 1 was located in a classroom at the opposite end of the building to the classroom where child 1 was currently in care.

Plan of Correction: The emergency medication for child 1 will be relocated so that is closer to where the child is in care. A list that is confidential will be provided to all staff who care for any child with allergies and or emergency medication. Going forward if a child in care who has allergies and or emergency medications is changing classrooms the information regarding the child's medication needs and allergy updated allergy list will be provided to the staff in advance of the child moving to the new classroom.

Standard #: 8VAC20-780-270-A
Description: Based on observation the center failed to ensure that areas and equipment are maintained in safe condition.
Evidence:
1 - There is an "super coffee energy drink" within reach of children, sitting on top of a cabinet approximately 3.5 feet tall in the school age. Energy drinks are not safe for children to consume and therefore should be left within the reach of children.
2 - In the two year old room the changing table cabinet door that is broken and hanging off one of the hinges. The broken door creates a pinch point.
3 - In the two year old room there is peeling paint within reach of children on a blue shelf.
4 - In the two year old room there is hole in the door that is approximately 4 inches in diameter and large enough for a child's to hand to fit in.

Plan of Correction: 1 - All staff will be reminded and retrained in the need for energy drinks to be stored in locations not accessible to children in care.
2 - The cabinet door will be repair or replaced in such a way that it no longer poses a potential hazard.
3 - Areas of peeling paint will be repaired or the equipment removed.
4 - The hole in the door wild be repaired or the door will be replaced.

Standard #: 8VAC20-780-330-B
Description: Based on observation the center failed to ensure required measurements of resilient surface depth and fall/zones lengths meet standards.
Evidence: There are two new pieces of playground equipment: one with moving parts, a bouncy teeter-totter and climbing dome.
1 - There is less than the required six inches of resilient surface under, around and into the fall/use zones of the climbing dome or the bouncy teeter-totter. The resilient surface depth is approximately four inches.
2 - There is less than six feet of fall/use zone surrounding the bouncy teeter-totter or climbing dome.
a. Additionally, the fall/use zones of the bouncy teeter-totter and climbing dome overlap each other and are obstructed by the chain link fence.

Plan of Correction: The added playground equipment and any playground equipment with moving parts or that children climb on will either have the required fall/use zone and proper resilient surface depth or the items will be removed from the playground until these adjustments can be made.

Standard #: 8VAC20-780-430-A
Description: Based on observation the center failed to ensure that furnishings are an appropriate size for the children using it.
Evidence:
The children in the two year old room are sitting in chairs that do not allow their feet to touch the floor. The height of the table requires tall chairs. Therefore, the table nor the chairs the children were sitting at for lunch and other group activities were not the proper size for the children using them.

Plan of Correction: The center will ensure that the table and chairs in the two year old room and all rooms are the proper size for the children in the room.

Standard #: 8VAC20-780-500-B
Description: Based on observation and interview the center failed to ensure that a nonabsorbent changing table or countertop designated for diapering or changing is used for children younger than three years old.
Evidence:
Staff 2 stated that pull-up diapers worn by the children in the two year old room are changed while children are standing up in front the toilet and not on a diaper changing surface.

Plan of Correction: All staff will be retrained and reminded that all pull-ups and diapers must change on a diaper changing surface and not standing up in front of the toilet.

Standard #: 8VAC20-780-560-F-4
Description: Based on observation the center failed to ensure that children three years and younger are not served foods that are considered to be potential choking hazards.
Evidence:
1 - All children two years and up were served hot dogs that were cut in half across the width of hot dog.
2 - Staff 1 nor 3 was aware that the hot dogs needed to be cut in half across the length of the hot dog to prevent potential choking hazards in young children.

Plan of Correction: Going forward all hot dogs will be cut in half lengthwise before they the served to children. All potential choking hazard foods will be properly prepared before the foods are served to children three years old and younger.
