Comments:
An unannounced monitoring inspection was conducted on-site June 28, 2022. The director was available during the inspection. There were 164 children present, ranging in ages from 6 months to 11 years, with 29 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and 10 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on a review of staff records and interview on June 28, 2022, the center did not ensure that each staff record reviewed contained a central registry finding within 30 days of employment.
Evidence: The record of staff #9 (hired 11/22/21) contained central registry findings dated 1/15/22. Administration confirmed the results were late.

Plan of Correction: Send emails to central registry when approaching 30 days.

Standard #: 8VAC20-780-70
Description: Based on a review of records and interview on June 28, 2022, the center did not ensure that each staff record contains all required documentation.
Evidence: The records of staff #2, staff #5, and staff #9 did not contain documentation of a health statement. Administration confirmed the information was not complete.

Plan of Correction: Corrected on site - director will double check future files

Standard #: 8VAC20-780-500-B
Description: Based on observation and interview on June 28, 2022, the center did not ensure that diapers shall be disposed in a leak-proof or plastic lined storage system that is foot-operated or used in such a way that neither the staff member's hand nor soiled diaper touches an exterior surface during disposal.
Evidence: Staff was observed, after diapering, disposing of a dirty diaper, in a trash can just outside the door leading to the outside area of the center, which was neither leak-proof or foot operated. The staff was observed using the staff's hand to lift the lid to the trash can and dispose of the diaper. The staff acknowledged the foot-operated pale was not used.

Plan of Correction: Corrected on site - staff was reminded to use touchless diaper pale.

Standard #: 8VAC20-780-510-L
Description: Based on observation and interview on June 28, 2022, the center did not ensure medication, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence: In the 3 year old classroom a staff's purse was located on a shelf accessible to children. There were several prescription medication bottles observed at the top of the staff's purse. The staff confirmed the purse was not in a locked cabinet.

Plan of Correction: Corrected on site- staff was told to lock up purse and personal belongings.
