Comments:
An onsite unannounced monitoring inspection was conducted on June 13, 2022. There were 27 children in care ages 22 months through four years old with six staff. A sample of children and staff records were reviewed. Children were observed in teacher and self-directed activities including but not limited to play activities, outside play, transitions, restroom routines, lunch and nap. Required postings, annual inspections, emergency evacuation drills and emergency equipment were reviewed. Violations were observed and appear on the violation notices and were reviewed with the administrator during the exit interview at the conclusion of the inspection.

Violations:
Standard #: 8VAC20-780-40-M
Description: Based on observation and interview the center failed to ensure that there is an accessible list of children's allergies that is dated and kept confidential.
Evidence:
1 - Child 1 who has a documented allergy to milk, was not included on the allergy list maintained in the two year old classroom.
a. Child was served milk with his lunch.

Plan of Correction: All allergy lists have been updated and will be monitored daily to ensure that they are always current.

Standard #: 8VAC20-780-50-A
Description: Based on observation the center failed to ensure that staff and children's records are treated confidentially.
Evidence:
The children's records were stored in an unlocked file cabinet in the hallway next to the entrance used by parents to enter and exit the center.
a. During the inspection there was not any staff in the area with the file cabinet.

Plan of Correction: Children's records have been moved to location where they will be locked and closely monitored.

Standard #: 8VAC20-780-80-A
Description: Based on record review the center failed to ensure that written attendance is maintained as required.
Evidence:
The attendance record in the two year old room lists 12 children however, 13 children were present during the inspection.

Plan of Correction: All staff have been retained and remind of the need for attendance records to maintained. Attendance documentation and procedures have been updated so that the attendance sheet in each classroom at any time during the day.

Standard #: 8VAC20-780-270-A
Description: Based on observation the center has failed to ensure that areas are maintained in safe condition.
Evidence:
There were three wooden cubbies, approximately four feet tall that are not secured to the wall and pose a toppling hazard.

Plan of Correction: All unsecured will be secured to a wall so that they no longer pose a toppling hazard.

Standard #: 8VAC20-780-410-6
Description: Based on observation the center failed to ensure staff do not use verbal remarks which are demeaning to the child.
Staff were overheard using degrading, annoyed and cross tones when speaking the following statements to children in care.
"Leave it where I put it."
"I'm not playing with you today"
"Be still stop moving around."
"Get that thumb out of your mouth now."
"Get up! Get up! Get up!"

Plan of Correction: All staff will be retrained in acceptable forms of behavior guidance.

Standard #: 8VAC20-780-500-B
Description: Based on observation the center failed to ensure that the diaper changing surface is used only for changing diapers.
Evidence:
Upon entering the toddler classroom there was a tray sitting on the diaper changing table. The tray held lunch foods that were served to the children.

Plan of Correction: All staff have been retrained and now understand that nothing can ever be stored on diaper changing surfaces.

Standard #: 8VAC20-780-520-B
Description: Based on observation the center failed to ensure that sunscreen is labeled with the child's name
Evidence:
There is one can of sunscreen in the two year old room that was not labeled with a child's name.

Plan of Correction: All staff will be reminded and retrained in the need for all sunscreen to be labeled with the child's name.
