Comments:
An unannounced monitoring inspection was conducted beginning at 10:50 a.m. and ending at 12:30 p.m. with the director. There were 153 children in care ages 8 months of age to 11 years old under the care and supervision of 17 staff. Infants were observed being fed and having playtime on the floor. Older infants and toddlers were observed having playtime. Two year olds were observed in the STEM lab working on an activity with blocks and computer lab. Three year olds were observed during music class and a painting activity. Four year olds were observed during transition time preparing for the gym. Five and six year olds were observed painting and having free play. Summer campers were observed having electronics time, playing on devices that are brought from home.

Seven children's records and five staff records were reviewed.

Information gathered during the inspection determined non-compliances with applicable standards or laws and violations were documented on the violation notice issued to the program.

Description: Based on review of documentation and interview with staff, it was determined that the facility failed to obtain an annual inspection with the Health Department as required. Evidence: The most recent inspection documentation was dated 5/10/2019.

Description: Based on observation, not all materials and equipment available to children were age and stage appropriate. Evidence: In the STEM lab, two years old were observed playing with small manipulatives that posed a choking hazard. One child was observed putting the small block in his mouth.
