Comments:
An unannounced monitoring inspection was conducted on Monday June 27, 2022 from approximately 12:05 pm to 1:25 pm. There were 32 children in attendance and a total of 5 staff present. 4 staff records and 5 children?s records were reviewed.

All areas of the center including classrooms, hallways, kitchen, bathrooms, and the playgrounds were observed and inspected. Infant bottles were labeled and dated correctly. Children were observed taking a nap.

Emergency drills and inspections were in compliance. The most recent Health Inspection was on 10/28/21. Fire Drills were conducted monthly-the most recent was on 05/31/22; Shelter in Place Drills were conducted twice-with the most recent on 01/04/22. The first aid kit had all components. The license, current inspection, and evacuation plan were posted, and emergency numbers were posted by each phone.

Violations were cited as a result of this inspection and are identified on the violation notice.

Standard #: 8VAC20-780-160-A
Description: Based on a review of records the center did not ensure that each staff member submitted documentation of a negative tuberculosis screening no more than 30 days prior to the first date of employment. Evidence: 1 out of 4 staff records had a TB test done more than 30 days prior to employment. Staff #1, date of employment 06/27/22, had a TB record with the date of 04/26/22.

Plan of Correction: Per the Director ?I will update the TB today.?

Description: Based on a review of records, the center did not ensure that two or more references as to character and reputation as well as competency were checked before employment for one of four staff. Evidence: The record of Staff #3, date of employment 09/20/21, did not contain any references.

Description: Based on a review of documentation, the center did not ensure to provide an annual fire inspection report. Evidence: The most recent Fire Inspection was dated 07/20/20.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a safe and operable condition. Evidence: 1) There were 3 missing wooden slats on the fence. 2) There were 2 broken wooden fence posts that were sharp. 3) There were several rusty and protruding nails on the wooden fence. 4) The chain link portion of the gate was not secured on all sides and was hanging off the post. 5) There was a rope that was hanging off of the play equipment that was not secured at the bottom as per manufacturer instructions.

Description: Based on observation, the center did not ensure that a fall zone was created free of hazardous obstacles. Evidence: The swing set with a height of 6 feet 9 inches was sitting directly beside the fence and had large toys less than 10 feet directly in front of and behind the trajectory of the swing and directly beside all sides of the slide/climbing wall/platform. The height of the swing set is 6ft 9inches. There is approximately 6 inches between the structure and the fence on side A, 7 feet 4 inches between the structure and the fence on side B, 11 feet 6 inches from the back of the fence to the structure, and 9 feet from the front of the structure to the wooden ground piece. Swings are to have a minimum fall zone of six feet on the sides of the structure, and twice the height of the swings in front of and behind the swings.

Standard #: 8VAC20-780-340-F
Description: Based on observation, the facility did not ensure that children under ten years of age were within actual sight and sound supervision of staff. Evidence: During the inspection, the licensing inspector observed the following:
1. At approximately 12:10 PM the licensing inspector was escorted by management to observe the Toddler, Two?s, and Preschool classrooms. Upon entering each classroom, the licensing inspector could not see the placement of the children on cots or the supervising staff member in the classroom due to all of the lights being turned off and no natural light being able to come through due to having no windows.
2. The licensing inspector addressed the lack of lighting with the Director. She agreed that it was too dark to see the children.
3. At approximately 12:30 pm the licensing inspector entered the infant classroom. One of the classroom doors was opened during the inspection to allow light into the classroom. Once the additional lighting was added to the classroom, the licensing inspector was able to see 7 infants resting on cots and in cribs and being supervised by two staff members.

Plan of Correction: Per the Director ?I will provide lighting by the end of today.?
