Comments:
A non-mandated inspection was conducted on June 30, 2022 regarding a self-reported incident that took place on May 23, 2022. The director was contacted by telephone and a virtual inspection was conducted. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, and special services. A total of two child records and two staff records were reviewed, and three staff were interviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-340-A
Description: Based on staff interviews and confirmation from staff, the staff failed to ensure the care and protection of one child.

Evidence:
1. In Classroom A, on May 23, 2022, Staff #1 gave a bottle of breastmilk labeled with Child #2's name to Child #1.
2. Staff #1 reported she removed a Ziploc bag of breastmilk from the freezer, thawed it, and poured it into a bottle labeled with Child #1's name. Staff #1 handed the bottle to Staff #2 to feed Child #1. Staff #2 verified that the bottle was Child #'1's before feeding the child. When Staff #3 was leaving for break she noticed the ziplock bag of breastmilk labeled with Child #2's name on the counter, and a pouch with Child #2's name in the trashcan. Staff #3 also stated that Child #1 breastmilk was still in the refrigerator and freezer. Staff #2 stated that Child #2's breastmilk had been brought into school that day in bottles, and that none of the reserve breastmilk was needed. Staff #1 confirmed that she did not look at the name on the ziplock bag when preparing the bottle for Child #1.

Plan of Correction: Administration met with staff and they have implemented a double verification process when preparing formula and feeding infants.
