The licensing inspector conducted an unannounced renewal inspection on Wednesday, June 15, 2022 from 10:42am to approximately 12:40pm. There were a total of nine children in care in the direct care of two staff members. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility used by the children were inspected. The center is equipped with toys and supplies and items were available to the children. The required postings were reviewed and found to be in compliance. During the inspection, five children?s records and two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Standard #: 8VAC20-780-60-A Description: Based on a review of five children?s records and interview, the center did not ensure two children?s record contained all the required information.
Evidence: 1) The record for Child #2, enrolled on 03/14/22, did not contain the address for the second emergency contact listed. 2) The record for Child #4, enrolled on 03/17/22, did not contain the address for the two emergency contacts listed. 3) During interview, a member of management confirmed the records did not contain all the required information. Each child record should contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
Plan of Correction: The missing information will be obtained on 06/15/22.
Standard #: 8VAC20-780-340-C Description: Based on a review of documentation, observations, and interviews, the center did not ensure that during the stated hours of operation, there always shall be on the premises and on field trips when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them.
Evidence: 1) During the inspection on 06/15/22, nine children were observed in the care of Staff #3 upon the inspector's arrival at 10:42am. 2) During interview, Staff #3 reported Staff #2 left the center at 10:30am at the end of her shift because Staff #1 was scheduled to arrive at 10:30am. It was reported that Staff #1 was running late and did not arrive at the center until approximately 12:45pm. 3) It was determined that Staff #3 was the only staff member present at the center with nine children from 10:30am until 11:45am when Staff #4 arrived. The children's ages ranged from two years to age 11.
Plan of Correction: Corrected on 06/15/22. In the future, the center will overlap staff to ensure there are always at least two staff on site while children are in care.
Standard #: 8VAC20-780-350-B-3 Description: Based on observations and interviews on 06/15/22, the center did not maintain the required ratios for children two years of one staff member for every eight children.
Evidence: 1) At approximately 10:42am, the licensing inspector observed nine children, ages ranging from two years to 11 years of age, present at the center in the care of one staff member. 2) During interview, Staff #3 acknowledged there was a lapse in staff coverage that morning and a second staff member would be arriving shortly. 3) Staff #4 arrived at the center at approximately 11:45am.
Plan of Correction: Corrected. The center had to combine the classrooms due to staffing issues. In the future, the center will overlap staff to ensure the appropriate ratios are maintained in the event staff call out or are running behind.
