Comments:
An unannounced renewal inspection was conducted on 7/06/2022 from 10:41am to 11:45am. At the time of entrance,12 children were in care with 3 staff members present. Children were observed doing the following: playing in centers. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, emergency supplies, evacuation drills and attendance records were reviewed. Standard #: 22.1-289.035-B-4
Description: Based on record review, the center did not have the results of a criminal history record check from any state in which the individual has resided in the preceding five years prior to the 1st day of employment.
Evidence:
1. Staff #1 (start date 7/06/2022) lived in Indiana in the last 5 years and the center did not have a copy of the results of the criminal history record check for that State in the file.

Plan of Correction: Criminal history record check for the state of Indiana was requested on 7/06/2022 for staff #1 (start date 7/06/2022) and results will be kept on file.

Standard #: 8VAC20-780-160-A
Description: Based on record review, the center did not have documentation of a tuberculosis (TB) test or screening for one staff member at the time of employment and prior to coming into contact with children and completed within the last 30 calendar days of employment.
Evidence:
1. Staff #1 (Date of hire 7/06/2022) was at the center working with children and did not have a TB test or screening on file.

Plan of Correction: TB screening was scheduled on 7/06/2022 for staff #1 (start date 7/6/2022). Results will be kept in the file.

Standard #: 8VAC20-780-160-C
Description: Based on review of staff records, the center did not obtain updated tuberculosis (TB)screening or testing every two years for one staff member.
Evidence:
1. Staff #2 had a TB screening or testing completed on 6/15/2020 and did not have an updated screening or test in the file.

Plan of Correction: TB screening was scheduled for staff #2 on 7/6/2022. Updated screening results will be kept in the file.

Standard #: 8VAC20-780-70
Description: Based on record review and interview, staff records did not include all required information.
Evidence:
1. Staff #1 was at the center during the inspection on 7/06/2022. The director stated it was Staff #1's first day. The center did not have the following information:
Name, address, verification of age requirement, job title, the date of employment, name, address and telephone number of a person to be notified in an emergency and two or more references.

Plan of Correction: All required information was put on staff #1 file by the end of her first day on 7/6/2022.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning products were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. A disinfecting cleaning spray was on the counter near the bathroom and was not kept locked.

Plan of Correction: A disinfecting cleaning spray was put to the locker during the inspection.
