Comments:
The licensing inspector conducted an unannounced monitoring inspection on 07/06/2022 in response to a self-reported incident that was received in the licensing regarding behavioral guidance. Interviews were conducted with staff, records reviewed and observations of the program were made relating to the allegation. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-410-1
Description: Based on interviews and a record review, the center did not ensure children were not handled roughly.

Evidence:
Staff A roughly handled babies in the infant room.
A. Staff witnesses and a parent witness reported seeing Staff A grab a one-year-old baby
(child 1) by the arm and drop her in the crib when the baby was crying.

B. One witness reported seeing Staff A slam the crib bars closed after putting the baby (child 1)
down in the crib.

C. Another witness reported seeing Staff A push the head of a baby down into the mattress of the crib.

D. Witnesses described Staff A as "aggressive" and "frustrated" with the infants in her care.

Plan of Correction: Staff A was placed on administrative leave. The District manager and licensing was contacted to partner in the investigation.
