An unannounced mandated inspection was conducted on 7/7/2022. There were 103 children, ages 1 month -12 years 11 months and 15 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 5 children?s records and 5 staff records were reviewed. The children were observed during lunch and naptime. The inspector arrived for the inspection at 11:45am and departed the center at 3:00pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-780-150-B
Description: Based on review of five children?s records, the center failed to ensure that all immunizations obtained were signed by the physician.
Evidence: The review of records for child #2 showed no immunization record signed by the physician.
1. Child #2 started on 3/22/2022 and on the day of the inspection did not have the required immunization record signed by the physician.

Plan of Correction: Center contacted guardian of child #2 and the physicians office will provide a signed copy by 7/25/2022.
