Comments:
A complaint inspection was conducted on July 11, 2022. Staff interviews were conducted and records reviewed. Areas inspected for compliance included administration, staffing and supervision, programming, medication, special care and nutrition. Information gathered during the inspection determined the complaint was valid. Non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-340-D
Complaint related: No
Description: Based on interviews with staff, in each grouping of children there was not at least one staff member who meets the qualifications of a program leader.
Evidence
Staff confirmed during an interview that child #1 was being switched often between the Toddler, and Preschool classrooms.

Plan of Correction: We will make sure there is a lead qualified staff member with each group of children.

Description: Based on interviews staff to children ratios were not always in compliance.

Evidence
1.On June 30, 2022 child #1 (DOB 5/17/21) was moved from the Toddler classroom to the Preschool classroom, then back to the Toddler room.

2. When child #1 joined the Pre School classroom, the number of children increased to 10 children with 1 staff member.

3. The ratio for 2 year olds (Toddler) is 8 children with 1 staff member and the ratio for 3 year olds (Preschool) is 10 children with 1 staff member.

4. The Pre School classroom was now out of ratio.

Plan of Correction: At our staff meeting on July 13, 2022 we reviewed ratios and our policies on ensuring we are always in ratio.

Description: Based on interview with staff, the center did not ensure that each group of children receives care by consistent staff or team of staff members.
Evidence
Children are not receiving care by consistent staff. Children are often being moved from one classroom to another.

Plan of Correction: We will not move children from classroom to classroom, unless they are in the process of transitioning to that classroom.

Description: Based on review of medication forms and centers procedures, the center is not following their own procedures for administering medication.
Evidence
The medication authorization form and allergy action plan was not completed correctly and did not have start and end dates for administrating medication.

Plan of Correction: Management will review all MAT forms to ensure they are complete before accepting any medication.

Description: Based on interviews it was determined a child with a diagnosed food allergy was served a food identified
in the written care plan.

Evidence
1.On June 30, 2022, food was placed on child #1?s plate that was identified in their written care plan.

2. Child #1?s plate was placed in front of them.

3. Staff #2 noticed what was on child #1?s plate and removed it.

4. Child #1 stated she did not eat any of the food.

5.Staff #1 who put the food on her plate, stated she does not think child #1 ate any of the food.

6. Staff #2 who removed the plate from child #1 stated the food did not look like it had been touched.

7. Child #1 was brought to the office for observation and parent was called.

Plan of Correction: All staff were retrained on our policies for allergies. A new allergy/food intolerance list has been posted in each classroom. All staff will review the form daily. When food is delivered to a classroom where there is a food allergy/intolerance it will noted so the food is not put on the child's plate.
