Discussed with the provider ensuring crib sheets are tight-fitting.

Comments:
An unannounced Monitoring Inspection took place on 07/11/2022 between approximately 10:05 a.m. and 11:30 a.m. There were 6 classrooms observed with a total of 42 children with 6 staff within the supervision guidelines. The children were observed completing a math activity, playing with blocks, napping, playing on the playground and other organized activities. Positive interactions between staff and children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-60-A-8
Description: Based on record review, the center did not obtain all information for children?s records.

Evidence: Child #3?s record indicates an allergy to egg, tree nuts and peanuts. The record for Child #3 did not include a written allergy care plan.

Plan of Correction: The center has requested the required form from the parents.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.

Evidence:
1. In the PreK-2 classroom, there was chipped paint located under the window in the reading area and by the window between the sensory and writing shelves.
2. In the Intermediate classroom, there were three spots of chipped paint by the window and chipped paint under the bulletin board.
3. In the PreK-1 classroom, part of the baseboard was detached from the wall in the block area.
4. On the Toddler/Beginners playground, there were four columns with chipped pain.
5. On the Toddler/Beginners playground, the rails for the stairs leading up to the slides had rust and chipped paint.
6. On the Intermediate/PreK playground, the rubber covering on the yellow swing was peeling off.

Plan of Correction: The center has put in a work order for all repairs to be made.

Standard #: 8VAC20-780-330-B
Description: Based on observation, where playground equipment is provided, resilient surfacing did not comply with the minimum safety standards.

Evidence:
1. On the Toddler/Beginners playground, mulch was missing at the bottom of the single red and double red slides.

Plan of Correction: The center has inquired with our landscaper when mulch is being delivered. Landscaper guaranteed mulch was being replenished.

Standard #: 8VAC20-780-520-A
Description: Based on observation, over-the-counter skin products were kept beyond the expiration date of the product.

Evidence:
1. In the Beginners classroom, there was a cream belonging to Child #1 that expired 09/2021.

Plan of Correction: Returned to parent. Monitor dates more closely.

Standard #: 8VAC20-780-520-C
Description: Based on observation, not all diaper creams were labeled with the child?s name.

Evidence: In the Toddlers classroom, there was a diaper cream belonging to Child #2 that was not labeled with the child?s name.

Plan of Correction: Label properly.
