Comments:
An unannounced monitoring inspection was conducted on 07/13/2022. There were a total of 53 children, ages 17 months- 12 years, and 10 staff members in attendance during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed during outside play and lunch. The school-aged children were on a fieldtrip during the inspection. A total of 5 children?s records, 5 staff records, and 1 medication were reviewed. The inspector discussed the following with the staff: diapering, allergies and outside play space. The inspector arrived for the inspection at 9:45 am and departed at 12:15 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-80-A
Description: Based on document review and discussion with staff, the center failed to ensure that for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: Review of the written attendance document for the Butterflies Classroom confirmed that the daily attendance record did not document the departure of each child as it occurred on 07/11/2022 and 07/12/2022.

Plan of Correction: Will review with the afternoon staff.

Standard #: 8VAC20-780-330-B
Description: Based on observation and discussion with staff, the center failed to ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.

Evidence:
1. The yellow and blue piece of playground equipment on the school-age side of the playground did not have resilient surfacing that complies with the minimum safety standards under and around the structures to create a fall zone. The piece of equipment was located on top of an outdoor carpet/artificial grass product which is not an approved resilient surfacing material.
2. The yellow, blue and red piece of playground equipment located on the Pre-k playground had a 2 foot 4 inch fall zone on one side; the required fall zone is 6 feet on all side of playground equipment. The fall zone for that piece of equipment did not comply with the minimum safety standards described in the American Society for Testing and Materials.

Plan of Correction: Will extend the border and get proper resilient surfacing.

Standard #: 8VAC20-780-550-P
Description: Based on document review and discussion with staff, the center failed to maintain a written record of children?s serious and minor injuries that contains all of the elements as required by the standards.

Evidence: The two injury reports that were reviewed did not contain the following required information: date and time when parent were notified; any future action to prevent recurrence of the injury; and documentation on how parent was notified.

Plan of Correction: Will review with staff.
