Comments:
An in person renewal study was conducted today. All rooms were in ratio during the inspection. Most children present were resting during the inspection. The playground (shade), first aid supplies, emergency supplies, diapering procedures injury reports and emergency evacuation drills are all in compliance. Violations and corrective action plans were discussed with the Director. See Violation notice for non-compliances. The inspection will be typed off site and emailed to the director for signatures. Standard #: 8VAC20-780-160-A
Description: Based on a review of records, one staff record does not contain documentation of an acceptable Tuberculosis screening.
Evidence:
1 of 3 staff records reviewed did not contain documentation of a TB screening result within 30 days of employment. The screening is dated 3/14/22. Date of employment is documented as 06/20/22. (Staff A)

Plan of Correction: Moving forward all TB screenings will be documented prior to 30 days of employment.

Description: Based on a review of records, child records do not include all required information.
Evidence:
3 child records were reviewed.
Missing information includes:
*Emergency contact address (Child 2)
*Documented viewing a Proof of Birth document (Child 2)
*No allergy action plan for documented food allergies. (Child 3)

Plan of Correction: I will speak to the parents to obtain the missing information. I will ask the parents to clarify if the child has allergies or intolerances. If it is an allergy I will have the parents provide an allergy action plan.

Plan of Correction: Moving forward orientation will be documented using the "Model form" listing all required topics provided by DOE.

Description: Based on a review of medications, one medication is not in the original box with a prescription label attached.
Evidence:
One medication is not in the original box, with a prescription label attached. (Child #1)

Plan of Correction: I will speak with the parents to obtain a new medication with the label attached.

Standard #: 8VAC20-780-580-B
Description: Based on observation there is no documentation available to ensure that the vehicle used to transport children meets the safety standards set by Department of Motor Vehicles.
Evidence:
The vehicle is registered in Virginia with Virginia license plates; however, no annual safety inspection has been conducted.

Plan of Correction: We have reached out to the corporate office and the leasing company to find out what needs to be done about this.

Description: Based on a review of the information kept in the transportation vehicle, all required information is not available.
Evidence:
The notebook used for the vehicle transporting children does not include documentation of emergency numbers, regional poison control .

Plan of Correction: The book will be updated to contain all required information tonight.
