An unannounced non-mandated inspection was initiated on 06/22/2022 and concluded on 06/23/2022. The purpose of the inspection was to investigate a complaint of an alleged incident that occurred while on a field trip on 06/21/2022 regarding supervision. One staff member was interviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-340-A Complaint related: Yes Description: Based on interview and verification, the center failed to ensure the care and protection of a child at all times.
Evidence:
1. On 06/21/2022, children were on a field trip at the local pool.
2. The staff allowed the oldest child in care to take a few children to the restroom while the staff were close by.
3. The staff verified that they sent the oldest child with the children to the restroom.Plan of Correction: I have told them that they could not allow this to happen again on any field trip. All children are to be within sight and sound supervision.
