Comments:
A renewal inspection was initiated on 7/18/2022 and concluded on 7/18/2022. There were a total of 86 children with 15 staff members present. Children were observed during snack time, engaged in group activities and outdoor play time. A total of 3 child records and 3 staff records were reviewed. Policy and procedures, fire and health inspections and medications were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 22.1-289.035-B-4
Description: based on a review of staff records, the center has not obtained a copy of the results of a criminal history record information check, from any state in which the individual has resided in the preceding five years.
Evidence: Staff #2 indicated on the sworn statement they have resided in the state of Texas in the past 5 years. There is no documentation that the center has obtained a copy of the Criminal History results from the state of Texas.

Plan of Correction: The director will complete the form for the criminal record request and submit accordingly.

Standard #: 8VAC20-780-70
Description: Based on a review of staff records and Director interview, complete staff records have not been kept regarding documentation of 2 or more references before employment.
Evidence: Documentation of completed references were not found in the following staff records; Staff #1, date of employment 3/29/2022 and Staff #2, date of employment 4/25/2022. The director stated that she did not document the reference information.

Plan of Correction: Director will call references and document the information.

Standard #: 8VAC20-780-240-B
Description: Based on a review of staff records, staff have not completed orientation training prior to the staff working alone with children and no later than seven days of the date of assuming job responsibilities.
Evidence: Staff #2, date of employment 4/25/2022, has not completed orientation training.

Plan of Correction: The director will conduct the orientation today.

Standard #: 8VAC20-780-510-E
Description: Based on a review of medications, long term prescription authorization for medication does not have written authorization from both parent and physician.
Evidence: The medication authorization form for Child # 1's long term Benadryl medication does not have written authorization from the physician.
There is no medication authorization form signed by the parent and physician for Child #2's long term Benadryl medication.

Plan of Correction: The parent will have the physician complete a new medication authorization form with all the proper signatures.

Standard #: 8VAC20-780-510-I
Description: Based on a review of medications, the medication administered by the facility does not contain the prescriber's instruction pertaining to dosage, frequency and manner of administration.
Evidence: Child #1 prescription Epi pen and Benadryl did not have instructions as to how to administer, how much medication to administer and the route of administration.

Plan of Correction: The parent will get written instructions from the physician.
The center will revise their form with all the required information or use the DOE medication authorization model form.
